That's why we created our Wellness Package, ensuring your loved one receives the highest standard of care, coordinated and coached by a Licensed Physiotherapist (PT).

Who is it for?
For elderly loved ones who:

  • Are still independent and want to stay active.

  • Need a safe, guided health routine.

  • Want to prevent falls and accidents at home.

  • Wish to feel more confident in their movements.

  • Prefer regular exercise with vitals safely monitored.


The Wellness Package is intentionally designed for seniors who are Independent to Moderately Independent (e.g., walk with assistance/devices) and focused on long-term maintenance and prevention. This ensures our remote coaching model provides the highest value to you and your loved one.

The Two Paths to Your Peace of Mind:

1. The Integrated Mobility Management Package

This is the most seamless and cost-effective path to peace. It is designed for families who already use or choose to use a Med-Easy Caregiver. Our licensed PT performs a crucial hands-on Home Visit (1x monthly) to assess, treat, and create a personalized PT-Guided Care Plan. We then leverage our own Med-Easy Caregiver to implement that plan daily, while our PT follows up with 3x monthly Tele-Rehab Consults for coaching and adjustments. This integration means guaranteed accountability, minimal stress, and the best value.

2. The PT Consulting Standalone Package

For families who have a loyal private caregiver or a trusted family member handling daily assistance, this package allows you to access our high-level clinical strategy without changing your staffing. Our licensed PT provides the same foundational service—the crucial 1x monthly Home Visit and the 3x monthly Tele-Rehab Consults—but focuses the coaching specifically on your existing caregiver or staff. We give your current care team the professional direction they need to succeed, while you receive the essential Monthly Clinical Report and expert advice.

Your Assurance of Quality: Eligibility & Focus

To ensure our unique Tele-Rehab and coaching model provides the highest benefit, the Wellness Package is intentionally focused on those we can help most effectively.

  • Our Focus: This service is ideal for Independent to Moderately Independent seniors who require long-term maintenance, fall prevention, and support for stable chronic conditions. If they can walk with a cane or require only standby assistance, this is a perfect fit!

  • Clinical Integrity: To maintain the highest standard, enrollment requires a valid Physician's Order for Physical Therapy maintenance or continued rehabilitation.

*Need More Intensive Care?

If your loved one requires intensive, daily hands-on treatment to reach the "Moderately Independent" level, we will guide you to our Standard Home PT Sessions. Once stabilized, we welcome them seamlessly into the sustained, low-stress support of the Wellness Package.
